He minimum distance between lines is 75 ft (23 m), and maximum is 200 ft (61 m)

Each and every airlines have given their own criteria to their pilots. there cannot be a general answer to this. for our company the mvr is 600 metres

ZERO. Today, with technological gadgets available - you do not require any visibility at all. In absence of such gadgets, I believe about 100 m is minimum
