Can't say about Mumbai, but in geneal a society should be fromed with 8 or more members. objects, acivities to be performed and bye laws of the society should be framed. Executive committe member alongwith the chair person to be designated.
A prescribed form for registration of a society can be had from theDistrict Registrar (Firm and Societies) includes all the above mentioned points. On its lines, alteration according you needs, may be made, neatly typed and signed by all the members must be submitted in the o/o of the D.R.
A very small book on Firms n socieities, which includes all laws must be had from any law-books store which will guide you for further actions to be taken after the formation of the society.
