Doesn't your co has official format for all sorts of leave? Typically you should follow this procedure when ever you want long leave ie more than 5 days.
1. First talk to your boss and get a nod from him;
2. Then, write a letter; Top right hand corner; your name, designation; department; date; Start the letter with Respected Sir; Sub: Body of the letter: Then Yours sincerely;
3. You should always take oral permission from your boss. If you just hand over leave letter without talking to him/her first and if he is in bad mood he may say no just for the heck of it; After saying no in writing it will be difficult for him to reverse it even if he wishes to do so.
