As of the 2008 Games in Beijing, the athlete with the most Summer Olympic medals is gymnast Larissa Latynina of the Soviet Union who participated in the 1956, 1960, and 1964 Summer Games. She won 9 gold, 5 silver, and 4 bronze for a total of 18 medals.
Gold Medals:

1956 - Team competition, All-around, Vault, Floor exercise
1960 - Team competition, All-around, Floor exercise
1964 - Team competition, Floor exercise

I guess china as a nation. individually i m not sure. i guess phelps won the max in swimming.
