Using a standard business letter format , state the loss and any specifications for time, date, location, etc. as your first paragraph, 'I am writing to advise you of the loss of...'
Use the second paragraph to explain the reason or circumstances of the loss and, if necessary any further description description of the lost items including model or serial numbers, if applicable and available to you, keeping it short and to the point. Mention or list anything that may be attached to support your report.
Start the third paragraph by stating what you want the recipient to do with the information necessary for them to contact you; and the last sentence is used to thank them for their time and/or their action.
