How can a leading national news like The Hindu publish a wrong photograph of a died person?

Just like a leading National News Paper The Indian Express published in 1971 Indo-Pak War that Gen. Tikka Khan is dead. Where as Gen Tikka Khan was alive.

It is rarest of the rare cases. It is done by a prankster. It is no fault of the newspaper. The relatives generally print obituaries of their relatives and News Paper do not verify in such cases whether the person making such a request is dead person's relative or not. It is definitely a crime and it would not be difficult for the police to track down the culprit.
